It is our objective to provide a calm and trusting environment while you are visiting us. We understand you are in pain and we want to help make this visit as smooth as possible and give you all the information you need to put your mind at ease. Most of your worries can be identified and treated with testing and referrals by our office. Our staff will send you out to the best facility to treat your needs as soon as possible. You can trust our staff to make sure that your concerns and requests get to our doctors. We are here to help so please give us a call if you have any questions.
Our billing department is experienced and can help with any questions you may have. We take most insurances including Workers Compensation, Medical, Medicare, Private Insurance, HMO, PPO, Tricare, and Triwest, however, please check with your insurance carrier to see if we are a preferred provider on your plan.
We ask all New Patients to please arrive 15-minutes prior to your scheduled appointment time. We also ask you to bring the following information:
- Government issued picture ID (if applicable)
- Insurance Card(s) (Primary and Secondary)
- Payment (if you do not have insurance)
- Medical Records (if available)
- All New Patient information filled out (4 pages)
- A list of current medications
- A list of all Drug Allergies
